is authorized to discharge from a facility located 15 miles north of Farmington, New Mexico, in, San Juan County, New Mexico to an unnamed intermittent stream in Waterbody Segment 20.6.4.98 NMAC from different Outfall locations, in accordance with this cover page and the effluent limitations, monitoring requirements, and
other conditions set forth in Part I, Part II, and Part III.
