Underground Injection Control Reporting Forms for Owners or Operators

7520 reporting forms for owners or operators

Owners or operators can select and download reporting forms from the list provided.  Completed forms should be submitted directly to UIC state primacy programs.  In non-primacy states, completed forms should be submitted directly to the regional EPA program.

Each file includes instructions for completing the form.

EPA is working to convert some of these reporting forms to webforms that can be completed online. Current online webforms include:
