Survey 123 Templates for Regional SDAMs

This page has Survey 123 templates that support the collection of field data for the Regional SDAMs. For more information, please visit the pages about Regional SDAMs or about how the Regional SDAMs were developed, or the the frequently asked questions about applying Regional SDAMs.  

People who have a Survey123 license may download these templates to create their own survey for data collection using the Regional SDAMs, if desired. Using this template to create an individualized survey will not result in data submission to the agencies.  

Note: Beyond providing this template for the voluntary use of end users of the Regional SDAMs, the agencies do not anticipate providing additional technical support for the development of field data gathering solutions at this time.

To create a survey with the template:

  1. Download the template to your computer.
  2. Open Survey123 Connect on your computer.
  3. Select “New Survey” and enter a survey title (default = Form 1). 
  4. Select “File” from the left sidebar and navigate to the Excel file that contains the survey template. 
  5. Select "Create Survey."
  6. Make modifications to suit your needs (e.g., add new fields you wish to include in data collection). Publish the survey.
