Web Standard: Contact Us Page

Definitions

A Contact Us page with contact information specific to the web area topic or application is required for all EPA public web content. When a web area is created in the WebCMS, a Contact Us webpage and accompanying Webform is automatically created.

Customizing the Contact Us Page

In the WebCMS, the Webform associated with the Contact Us page must be edited to include an email address which will receive web inquiries. Content owners should customize the Contact Us Page.

Content Requirements

  • The page title should be Contact Us About (Topic/application name).
  • Contact Us pages must include an online form for sending questions or comments. (Alternatively, clearly linking to an online form on another page is allowed.)
  • Contact Us pages that receive public queries must be connected to the public query tool in Quill (VPN required).
    • Public queries received through Contact Us pages and all subsequent responses must be managed in the public query tool in Quill.
  • If inquiries collected through a Contact Us form are managed by an agency contractor instead of an EPA employee (or group of employees), the following statement must be added to the Contact Us page above the webform, "Information submitted to EPA via this form may be routed to an EPA contractor for processing. Please do not submit any sensitive personally identifiable information or other sensitive information via this form."
  • Physical mail addresses are optional but should include a link to the main Agency Mailing Address webpage.
  • Alternate contact information on the Contact Us page is required. Not everyone wants or is able to fill out a form. This alternative contact information may be a physical mailing address, a group email box or a telephone hotline rather than a specific person. If email addresses are provided, they must be EPA official email addresses (e.g. @epa.gov, @energystar.gov).
  • Webforms on Contact Us pages must allow for anonymous comments. Do not make the name or email address fields on the form required elements.
    • However, the form should make it clear: if the commentor does not provide an email address, they will not get an answer.
  • After submitting a form, the user must be directed to a separate confirmation page. This “thank you page” is automatically generated by the WebCMS, but you can modify the text.

About this Standard

All EPA public web content must adhere to all federal requirements including; the EPA Web Standards, the U.S. Web Design System guidelines, federal plain language guidelines, and the 21st Century Integrated Digital Experience Act. Internal content, including Intranet and Microsoft 365, may differ from the public content. If not explicitly stated, content must adhere to all federal requirements.
